We believe that the health of the working population is critical to the health of the community.  In order to live well, it’s important to work healthy. 

To that end, we strive to provide the highest level of patient care and client satisfaction.

We adhere to the practice guidelines of the American College of Occupational & Environmental Medicine.  Also, where applicable, we also utilize guidelines from the Official Disability Guidelines (ODG), the State of California Medical Treatment Utilization Schedule (MTUS), and the AMA Guides to the Evaluation of Permanent Impairment, 5th Edition

New injuries (first time seen) are on a walk-in basis. Follow-up appointments will be made as the patient leaves the facility after their visit.

REQUIREMENTS TO SCHEDULE AN APPOINTMENT

Federal guidelines require any medical provider to send a Doctors first report to the Workers Comp insurance company ─ even if the injury is deemed to be 1st aid eligible.

Every employer must provide Adventist Health Physicians Network with their Workers Comp insurance information. 

For injuries, please fill out a treatment authorization form. You can fill online the authorization form you will find below, print and bring it to your visit.

 

WHAT TO BRING WITH YOU

Please plan to arrive at least fifteen minutes early for your first visit to complete necessary registration form. You can fill online the registration form you will find below, print and bring it to your visit.

Please bring with you the following documents:

  • The treatment authorization form
  • List of current medications
  • Photo ID.

BILLING

We will bill to your Workers Comp insurance company directly.
