Please call us at 818 265-2242 to schedule an appointment. Your first visit with us is usually the longest and we leave the most time in our schedule for it. Please plan to arrive at least half an hour earlier for your first visit to complete necessary registration forms.

Photo identification - Adult patients must have photo identification, such as a driver’s license, passport, military ID, employment badge, or student identification card.
Proof of insurance - If you plan to pay for a non-work-related injury or illness with insurance, please bring proof of insurance.
Any insurance copay is due at the time of your visit. We make paying for your visit easy for you by accepting cash, personal checks, and major credit cards.
